This notice explains how your health information may be used and shared, and your rights regarding that information.

Our Privacy Responsibilities

We are required by law to protect the privacy and security of your protected health information (PHI), give you this notice, and follow the practices described here.

How We May Use and Share Your Information

We may use and share your health information without written authorization for:

  • Treatment: Providing dental care and coordinating with other health care providers
  • Payment: Billing and collecting payment from insurance or other payers
  • Health Care Operations: Running our practice and improving quality of care
  • Legal and Public Health Purposes: As required by law or to protect health and safety

2026 Updates

Federal law includes updates that provide additional privacy rights related to certain types of information.

  • Reproductive Health Information: We will not use or disclose reproductive health information for investigations or legal actions related to lawful reproductive health care, unless required by law or you authorize us in writing.
  • Substance Use Disorder (SUD) and Other Sensitive Information: Certain information, including substance use disorder records, mental health information, HIV/AIDS, and sexually transmitted infections, receives additional protection under federal and Washington State law. Some disclosures require your consent, certain counseling notes have extra protection, and these records generally cannot be used in legal proceedings against you without specific authorization or a qualifying court order.
  • Fundraising Communications: We may contact you for fundraising purposes as allowed by law. You have the right to opt out of receiving fundraising communications at any time. Your decision will not affect your treatment or payment for services.

If unsecured protected health information is breached, you will be notified as required by law.

  • Get a copy of your health records
  • Ask us to correct your records
  • Request confidential communications
  • Ask for limits on certain uses or disclosures
  • Receive a list of certain disclosures
  • Get a paper or electronic copy of this notice at any time

We follow Washington law when it provides greater privacy protection than federal law, including the Washington Uniform Health Care Information Act (RCW 70.02).

Questions or Complaints

If you have questions or concerns about your privacy rights, you may ask to speak with our Privacy Officer by calling our main office phone number. You may also file a complaint with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
